Sudden Cardiac Death Poses Significant Risk to Male Bodybuilders, Study Finds

A new study published in the European Heart Journal reveals that sudden cardiac death (SCD) accounts for a disproportionately high number of deaths among male bodybuilders worldwide, with professional bodybuilders facing the greatest risk.

Sudden cardiac death refers to an unexpected death caused by heart problems, which, although rare in young healthy individuals, is often linked to underlying heart conditions.

Led by Dr. Marco Vecchiato of the University of Padova, Italy, the research analyzed data from over 20,000 male bodybuilders who competed internationally between 2005 and 2020.

The study identified 121 deaths, with an average age of 45 years, and found that 38% were due to sudden cardiac death. Professional bodybuilders had more than five times the risk of SCD compared to amateurs.

Autopsy reports, though limited, frequently showed heart enlargement or thickening and coronary artery disease. Toxicology reports indicated anabolic steroid abuse in some cases.

Dr. Vecchiato explained that intense training, rapid weight loss, dehydration, and widespread use of performance-enhancing drugs place considerable strain on the cardiovascular system, increasing the risk of irregular heart rhythms and structural heart changes.

The study highlights the need for greater awareness and preventive measures within the bodybuilding community. Dr. Vecchiato emphasized the importance of safer training practices, medical supervision, and a cultural shift away from performance-enhancing drugs.

He also called on clinicians to implement proactive cardiovascular screening and counseling for bodybuilders, even young and apparently healthy ones.

Beyond physical health, the research noted that about 15% of deaths were sudden traumatic deaths, including accidents and suicides, underscoring the psychological pressures in bodybuilding culture, often exacerbated by substance abuse.

The findings do not condemn strength training or fitness but warn against extreme practices that jeopardize heart health. The researchers plan to extend their study to female bodybuilders and examine trends over time to better understand evolving health risks.
